Beskrivning av organisationen

The Healthy City Foundation (FZM) is a NGO founded in 2015 in northeastern Poland, active the areas of public health, education, and social integration. FZM's mission is to empower local communities through competence development, access to social services, and preventive measures. FZM conducts comprehensive educational, social, and counseling activities aimed at children, youth, adults, seniors, people with disabilities, and those at risk of social and digital exclusion. Particular emphasis is placed on groups with limited access to support, including residents of smaller towns and rural areas. FZM's activities are integrated and long-term – combining direct support (counseling, workshops, outreach activities) with the development of social, digital, and civic competencies. Projects are implemented in cooperation with local government units, educational institutions, healthcare entities and social organizations, which ensures their high quality, durability and real social impact. FZM has extensive experience in coordinating complex, cross-sectoral initiatives. It successfully initiates and maintains collaboration within structures such as the medical cluster and the energy cluster, and also implements R&D projects, preventive programs, and international initiatives, including those funded by Interreg programs. It is a co-founder of the Federation of Institutions for Continuing Education of the Society (FIKUS), which demonstrates its ability to build lasting partnerships.
